New to the ACCESS Center? Welcome!

To request accommodations from the City Colleges of Chicago, you are required to complete both the Student Intake Form and upload your documentation using the link below.

Once submitted, your home campus ACCESS Center will review your intake and documentation within 5-7 business days. When both items have been approved, you will receive an email to your CCC student email with instructions on how to schedule an intake appointment with your home campus AC Director.

If you have not heard from the ACCESS Center after 7 business days, please contact your home campus ACCESS Center for assistance.
